Ophthalmology relies heavily on diagnostic testing. Proper billing requires understanding what each test does and when it’s medically necessary.


🔹 1. OCT (Optical Coherence Tomography)

Types:

  • 92133 – Optic nerve (glaucoma)
  • 92134 – Retina (macula diseases)

Why it’s done:

  • Glaucoma monitoring
  • Macular degeneration
  • Diabetic retinopathy

👉 Must include interpretation & report or it won’t be paid


🔹 2. Visual Field Test

  • 92081–92083

Purpose:

  • Detect peripheral vision loss
  • Monitor glaucoma

🔹 3. Fundus Photography

  • 92250

Purpose:

  • Document retinal conditions
  • Track disease progression

🔹 4. Pachymetry

  • 76514

Purpose:

  • Measure corneal thickness
  • Important for glaucoma evaluation

🔹 5. Gonioscopy

  • 92020

Purpose:

  • Examine drainage angle (glaucoma)

🔹 Billing Essentials

  • Must have medical necessity + diagnosis
  • Physician order required
  • Separate interpretation report mandatory
